A Case of Fibrin-ring Granuloma in Bone Marrow and Liver Associated with Epstein-Barr Virus Infection



Abstract
Fibrin-ring granuloma is a peculiar type of granuloma described in liver and bone marrow of several disease ; Q fever, Hodgkin's disease, CMV hepatitis, Hepatitis A, and allopurinol-induced hepatitis. In the present study, we report one case of
fibrin-ring granuloma in a patient with chronic Epstein-Barr virus (EBV) infection. A 45-year-old man with chronic Epstein-Barr virus infection and EBV hepatitis was demonstrated by serology and molecular biologic study including PCR and in situ
PCR.
His bone marrow and liver showed characteristic multiple fibrin-ring granulomas with central vacuole surrounded by a dense fibrin ring. There was no evidence of Q fever, neoplastic disease, drug-induced hypersensitivity or presence of other
infectious
agents. So EBV was a sole etiologic agent of fibrin-ring granuloma in this patient and EBV infection should be considered in differential diagnosis of fibrin-ring granuloma in liver and bone marrow (Korean J Clin Pathol 1995;15(2) : 178~82).
